Manhunters origin – Surgeon does not save lives

I wanted to try surgeon retinue on my manhunter origin run. He did not save my indebted borthers lives (I assume this is intended) but he did also not save my non-indebted non-permanently injured brother’s life.

Not sure why but I could not add them directly on the forum, adding attachments did not work for me.
Notice Yazid had no permanent injuries so I would expect Surgeon to save his life with perma injury.
