Enemies that die from bleeding or poison don’t provide XP. This is an old well-known feature.
Loot drops from enemies who died from bleeding (even from bleeding injury) or poison, but not in the case of beasts.

I would call this a mechanic feature, not a bug. The Barbarian Reaver first aimed at the brother without a shield, otherwise the chances of hitting are not used and the damage is dealt only to the shield. If the damage was not originally planned to be applied to the shield, then the spear works in the same way as throwing javelins/axes/bolas.

I’m sure the location spawned on the map, you just didn’t find it, because the visibility of the Icy Cave is in the range of 80% of your general vision and can be further reduced depending on the terrain (forest, mountains).
